Social Security Card Replacement: Documents Required

Use this checklist to prepare the original documents typically required for a replacement Social Security card (lost, stolen, or damaged) when applying by mail.

Identity document (choose one)

  • U.S. driver’s license
  • State-issued non-driver ID
  • U.S. passport

If you do not have one of the IDs above

The SSA may accept other current documents that show your name plus identifying information (for example, date of birth or age). A recent photo is preferred when available.

  • U.S. military ID (active, retired, National Guard, or dependent)
  • Certificate of Naturalization or Certificate of U.S. Citizenship
  • U.S. Indian Tribal Card
  • U.S. government employee ID (for example, Form OF-55)
  • School ID card (name, date of birth, and photo)

Important reminders

  • Mail original documents; photocopies are typically not accepted.
  • Your ID documents and your Social Security card may arrive in separate mailings.
